PARENTS, PLEASE READ:

All Lasertag participants must have a valid waiver on file with Liberty Arena! They encourage all to complete the waiver ahead of time. The Waiver to be signed can be found here: https://libertyarena.centeredgeonline.com/waivers

BEFORE DROPPING YOUTH OFF, you must make sure your youth gives their name to the clerk, so the clerk can check them in and verify that the youth has a waiver on file with Liberty Arena.  If your youth has not done Jumping or Lasertag in the last 3 months, their waiver will need to be updated.

Permission slip required for any youth not accompanied by their parent. If you are not attending with your child, you will need to have completed this Permission Slip / Hold Harmless agreement for your child to attend.
